How to Prepare Your Exams for Dropoff

Once you receive a confirmation email from our office that you have been successfully entered into our system. The next steps to prepare for your exams to be dropped off are as follows.

Classroom Exam Scoring Request Form

The first step is to fill out a  Classroom Exam Scoring Request Form. The form is a fillable pdf. You must completely fill in the pdf and place it with your materials.
For further instructions on how to complete the Classroom Exam Scoring Request Form, check out this instructional video!

Answer Key
After the Classroom Exam Scoring Request Form is completed you will then want to make sure that the Answer Key is clearly marked (in pencil only). Then you can place the key on top of all student answer sheets that you want to be scored.

Student Answer Sheets
Once you have the Key filled out, gather all of your student sheets (all facing the same way) and compile them in the envelope you plan to use to drop off your exams with. (Note: If you are submitting an alternate form exam, also include an Alternate Form Designation Sheet).
Once the envelope is packed exactly as instructed above – you may drop it off in the mail slot of Room 715A Carnell Hall.

How to Prepare an Alternate Form Dropoff

Some instructors use alternative forms to scramble test items and therefore help prevent cheating. Here is a copy of the Alt-Form Designation Sheet which you need to bring completed if you plan on using this feature (Alternate Form Designation Sheet PDF)

Form Construction

  • There may be up to four forms of one exam. (i.e. One Primary Key & 3 Alternates)
  • One form must be designated the "Primary" form and the others must be designated as “Alternate Form”
  • The same items (questions) and responses are used for all forms; however, the order of the items will vary between the forms of the exams.
  • The order of the response options must be the same for each item on all forms. Sample Student Sheet. For example, the answer to Question 1 on the Primary Form is A. Question 1 appears as question 10 on the Alternate Form and the answer must also be A

Form Designation on Answer Sheets

  • The test code (“11 for the primary form and “22”, “33”, etc. for the alternate forms) must be indicated in the two columns of the “TEST CODE” section found in the upper right-hand column of each answer sheet.
  • Required Information for Processing (i.e. Professor Name)
  • The answer sheets for the primary and alternate forms must be separated before the exam is turned in for processing. For example, group all answer sheets that use the primary form together and all answer sheets that use the alternate form together using a paper clip.
  • The instructor must prepare scoring keys for each form. Prepare one key indicating the correct answers for the primary form and a separate key for each alternate form. All keys must be clearly identified. You can achieve this by making sure you follow step one and correctly fill in the test codes for the corresponding form.
